West Elgin Community Health Centre is committed to working in partnership with individuals, groups and families to optimize the health and well-being of the residents and communities of West Elgin.

Community Health Centres are non-profit, community-governed organizations that provide primary health care, health promotion and community development services, using multi-disciplinary teams of health providers. Our teams include physicians, nurse practitioners, nurses, dietitians, health promoters, chiropodist, counselors, registered dietitians, community health workers, an early childhood educator.
We provide a variety of health promotion and illness prevention services which focus on addressing and raising awareness of the broader determinants of health such as employment, education, environment, isolation and poverty.
While you need to be a registered client to see a physician or nurse anyone can access our health promotion and mental health services and programs.

Primary Care Services

  • Health Assessment
  • Clinical evidenced-based illness prevention and health promotion
  • Appropriate interventions for episodic illness & injury
  • Primary reproductive care
  • Early detection of initial and ongoing treatment of chronic illness
  • Care for the majority of health conditions (in conjunction with specialists as needed)
  • Education and support for self-care
  • Supportive Housing care for seniors
  • Arrangements for 24-hour/7-day a week on-call availability
  • Service coordination and referral
  • Maintenance of a comprehensive client record for each consumer in the centre
  • Primary mental health care including psycho-social counseling
  • Coordination and access to rehabilitation
  • Support for people with a terminal illness

Community Support Services for Seniors

West Elgin Community Health Centre provides a number of support services for seniors and disabled community members. These services enable clients to remain independent in their own homes as long as possible.

With funding from the Long Term Care (LTC) Branch of the Ministry of Health, and with the assistance of over 100 community volunteers, this program provides the following Community Support Services for seniors.

  • Congregate Dining
  • Volunteer Transportation
  • Telephone Reassurance
  • Friendly Visiting
  • Caregiver Support - Education and Information
  • Supportive Housing

Four Counties Health Services(FCHS) is a small modern hospital with 24 hour ER, inpatient services, and visiting specialty clinics. There is no obstetrical care and major trauma is sent into nearby London.

Located in Four Counties Health Services, the Four Counties Medical Clinic currently contains 3 physicians and 2 nurse practitioners. A relatively new group practice, the principle physicians are currently exploring the opportunity to form a Family Health Group and are now looking for new partners to assist with their busy family practice.
